SouthEast Alaska Regional Health Consortium (SEARHC) is seeking a senior manager to oversee the Care Coordination and Discharge Planning team at Mt. Edgecumbe Memorial Center.

You will lead a staff of care coordinators and social workers, ensuring timely patient progression and resource access. The role requires robust nursing leadership and a deep understanding of post-acute care, Medicare/Medicaid, and regulatory compliance.

Qualifications

• Bachelors degree required; advanced degree preferred.
• Current, unrestricted RN license in Alaska or other US state required.
• Direct hires must apply for Alaska RN license prior to start date.

Job description

Pay Range:$50.14 - $70.35 This position oversees the coordination of inpatient care, Case Management/Discharge Planning, and Social Services for Mt. Edgecumbe Medical Center (“MEMC”). The Manager leads a team of Care Coordinators, Social Workers, and Inpatient Care Coordinator Technicians by providing leadership, managing operations, and ensuring quality of care. Key Essential Functions and Accountabilities

• Provide team leadership and supervision while fostering a collaborative and supportive environment.
• Manage daily assignments and schedules to ensure a balanced workload.
• Mentor new staff and provide ongoing team training as needed.
• Build and cultivate relationships with post-acute providers through active participation in community networking.
• Ensure quality of care provided by the team; monitor performance and implement changes to improve service delivery and efficiency.
• Collaborate with leadership to implement new strategies, set goals, and align departmental operations with the organization’s mission and vision.
• Support case managers and social workers in resolving complex cases and ensuring timely access to appropriate resources.
• Meet and exceed departmental service expectations; develop action plans to achieve established goals.
• Identify areas for improvement and implement corrective actions and education.
• Work closely with the Utilization Review (UR) team to maximize revenue while maintaining compliance with regulations. This includes knowledge of Medicare, Medicaid, and other third-party payers.
• Collaborate with administration to ensure appropriate hospital resource utilization. Internal coordination includes SEARHC Primary Care Clinics and Long-Term Care facilities; external coordination includes tertiary care, assisted living, and psychiatric facilities.
• Direct the Care Coordination Department to ensure safe, efficient, and effective use of hospital services and facilitate timely discharges.
• Maintain compliance with all regulations.
• Participate in the development and implementation of departmental policies and procedures; monitor for consistency and compliance.
• Oversee care coordination processes to ensure timely, appropriate patient progression and discharge planning.
• Collaborate with the System Director to review and revise workflows and procedures to optimize resources and enhance customer service.
• Monitor quality measures; collect and analyze data to improve service utilization and support service effectiveness.
• Ensure timely and appropriate documentation of services and progress consistent with care plans.
• Conduct internal audits to ensure compliance with organizational, state, and federal regulations.
• Coordinate with internal and external stakeholders, including primary care, long-term care, assisted living, and tertiary
• Promote efficient hospital resource utilization and patient throughput
• Collaborate with the Utilization Review (UR) team to ensure appropriate level of care, maximize reimbursement, and maintain regulatory
• Collaborate with leadership to establish departmental goals and align services with organizational priorities
• Participate in the development, implementation, and monitoring of policies and procedures
• Analyze data and trends to improve service delivery, efficiency, and patient outcomes.

Education, Certifications, and Licenses Required

• Bachelor’s degree from an accredited institution required; advanced degree preferred.
• Current, unrestricted Registered Nurse license in Alaska or another U.S. state required.
• Direct hires must apply for an Alaska RN license prior to the start date.
• Agency staff must hold an active Alaska RN license.
• Licensed Vocational Nurse (LVN) in Alaska or another U.S. state may be considered.
• Same licensing requirements apply as above.
• High school diploma or equivalent required.
• Clinical Competency required within 3 months of hire and every three years.
• Manager Competency required within 3 months of hire and every three years.
• ACMA Certification preferred.

Experience Required

• Minimum of 5 years of discharge planning experience.
• Previous management experience in supervision or administration required.
• Experience in a Critical Access Hospital model preferred.
• Minimum of 5 years of clinical experience.
• Experience in performance improvement preferred.

Knowledge of

• Post-acute care resources and Medicare/insurance coverage.
• Healthcare systems and federal/state regulations.
• Admission and discharge processes.
• Relevant regulations, best practices, and data management systems.
• Deadline responsiveness, assertiveness, persistence, and persuasiveness.
• High proficiency in problem identification and care plan development.
