Job summary

SCAN is a nonprofit health organization seeking a Nurse Practitioner leader to manage programs and clinicians delivering primary care within an integrated care delivery model. You will supervise clinical teams, ensure ethical and high-quality patient care, and develop on-boarded clinicians across specialties.

Requirements include a Master’s in nursing, Oregon RN/NP licensure, specialty certification, and 5+ years of NP experience with 3+ years of supervisory experience.

Qualifications

  • Master’s degree from an accredited school of nursing.
  • Oregon RN, NP, and Furnishing licensure required.
  • Certification in area of specialty required.
  • 5+ years of recent NP experience required.
  • 3+ years supervisory experience preferred.
  • Experience working with Medicaid and Medicare.

Responsibilities

  • Manages programs and clinicians providing primary care to SCAN members within the integrated care delivery model.
  • Oversees clinicians in daily operations, ensuring ethics and quality in patient care by Advanced Practice Providers.
  • Selects, on-boards, trains and develops clinicians within the integrated care model.
  • Provides clinical care including exams, diagnostics, care plans, and patient education.
  • Collaborates with senior management on operational and performance issues and new plans.
  • Develops and monitors success metrics and adjusts programs to improve efficiency.
  • Networks with internal and external stakeholders to promote growth and quality outcomes.
  • Maintains professional knowledge through education and professional societies.
  • Maintains confidentiality and supports SCAN’s vision and goals.
  • Supervises/Manages Others and performs other duties as assigned.

Selects, on-boards, trains and develops clinicians within the integrated care delivery model.Accomplishes staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching and counseling employees; initiating and enforcing systems, policies, and procedures.Provides clinical care to include performing exams, ordering diagnostic and screening tests, developing plans of care, accurately documenting patient medical history and care, and educating and consulting with patients and families.Collaborates with senior management on the resolution of operational and performance issues and the development of new plans and programs. Improves and integrates APP functions and systems across specialties and departments, including the development and implementation of APP Best Practice Models.Develops and monitors success metrics and adjusts programs as needed to improve efficiencies and effectiveness.Networks and builds relationships with internal and external key stakeholders to promote membership growth and quality outcomes for integrated care delivery products.Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; and participating in professional societies.Maintains client confidence and protects operations by keeping information confidential.Contributes to team effort by identifying and achieving goals/targets as needed.Actively support the achievement of SCAN’s Vision and Goals.We seek Rebels who are curious about AI and its power to transform how we operate and serve our members.Supervises/Manages Others.Other duties as assigned.Your QualificationsMaster’s degree from an accredited school of nursing.Oregon RN, NP, and Furnishing licensure required.
