Responsibilities

The Manager of Case Management provides onsite leadership to ensure daily department operations function effectively and efficiently to meet the needs of all customer and regulatory groups. You will act as a consultant to ensure the severity of illness and intensity of services interval criteria evaluations are completed on all patients as designated by Case Management goals. You will ensure that case managers assess, plan, and develop plans for patients based on recognized standards of practice for case management. You will support a culture of continuous quality improvement.

Qualifications

• Associate Degree in Nursing from an accredited school of nursing is required.
• Bachelor's Degree in nursing from an accredited school of nursing is preferred.
• If a Social Worker, a Master's Degree in Social Work from an accredited school of social work is required.
• Three (3) years of experience in a hospital setting as an RN or Social worker is required.
• Two (2) years of leadership experience is preferred.
• Case Management experience is preferred, but not required.
• Current RN license in the State of Indiana is required for nursing candidates.
• Current LSW or LCSW in the State of Indiana is required for social work candidates.
• Understands the relationship of individual patients, units, and/or departments to the network, to facilitate achievement of optimal goals with those relationships.
