Job summary

Methodist Richardson Medical Center is seeking a Care Management leader to oversee day-to-day operations of the Care Management Department, ensuring that resource use aligns with quality outcomes and staff development.

You will mentor Care Transition Navigators, plan and implement operational, financial and personnel activities, and provide guidance to prudent practice within the team. This role emphasizes collaboration and patient-centered care.

Qualifications

• Master’s degree from an accredited school of social work or Bachelor’s degree from an accredited school of nursing.
• LMSW licensed by the Texas State Board of Social Worker Examiners or RN licensed by the Texas State Board of Nursing.
• A minimum of five years of clinical practice.

Your Job:

Retains and develops competent staff. To assist in the planning, directing and implementing of operational, financial and personnel activities of Care Transition Navigators in the Care Management Department and provide guidance in prudent practice

Your Job Requirements:

• Master’s degree from an accredited school of social work or Bachelor’s degree from an accredited school of nursing
• LMSW as licensed by the Texas State Board of Social Worker Examiners or RN as licensed by the Texas State Board of Nursing
• A minimum of five years of clinical practice.
