Job description

The Director of Population Health is responsible for the direction and leadership of operational, financial, programmatic and personnel activities for Population Health.

This includes establishing, meeting and continuously monitoring the goals and objectives while maintaining alignment with the strategic goals and objectives to identify the need for a plan for improvement.

Responsibilities

• Work directly with CMO and operations team to launch a comprehensive population health program
• Identify and maximize risk assessment through identification of prior conditions, new conditions, and suspect conditions
• Work with team of coders to optimize risk scoring
• Improve quality measures and closure of existing care gaps to optimize care and maximize incentive payments
• Coordinate flow of care plans to Quality Care providers for development of care plans
• Oversee care managers in execution of customized care plans
• Engage PCP to utilize standardized care plans
• Facilitate and foster team approach between quality team, coders, care managers, EMR specialists, and PCP to ensure highest level of care for each patient

Skills

• At least 4 years of experience in health care population health, quality improvement and operations
• At least 4 years of experience inPopulation Health assessment and strategy with a demonstrated experience in Health Equity disparity analysis and improvement.
• Able to organize information and effectively present specific content tailored to different audience needs
• Experience working with data and analytics to support program design, effective outcome measurement and performance improvement
• Able to assess data sources for reliability and validity and use that information to balance inputs to a decision; able to balance hard and soft data to reach conclusions and make recommendations
• Master's degree in health care administration or related field
