Job summary

United Wound Healing in Puyallup, WA is seeking a detail‑oriented Care Coordinator to join our fast‑paced wound care team. You will support CCM/PCM programs, coordinate referrals and scheduling, and help patients and families navigate care pathways.

The role offers daytime hours, no nights or weekends, and a starting pay of $29.00–$31.00 per hour (DOE) plus benefits. Active WA licenses (MA, LPN, RN) preferred; strong communication and organization essential.

Qualifications

  • Active MA, LPN, or RN license/certification in Washington State.
  • Computer savvy with MS Office (Word, Teams, Outlook, Excel).
  • Strong verbal and written communication and professional phone etiquette.
  • Excellent attention to detail, time management, and prioritization.

  • Ability to multitask in a fast-paced healthcare environment.

Responsibilities

  • Maintain care coordination dashboards and patient lists for CCM/PCM.
  • Track referrals, authorizations, and follow-up across services.
  • Conduct non-clinical patient and family outreach, including welcome calls.
  • Coordinate communication between patients, facilities, Care Managers, and providers.
  • Gather, upload, and organize medical records, wound photos, labs, and imaging reports.
  • Support scheduling for follow-up visits, interdisciplinary rounds, and provider appointments.
  • Complete clerical tasks for Care Management, including outreach and documentation.
  • Monitor admissions, discharges, and census changes from facilities.
  • Prepare patient summaries and materials for care team meetings.
  • Escalate clinical concerns promptly to a Care Manager or provider.
  • Participate in required training and compliance activities.

Salary: $29.00 - $31.00 hourly (DOE) | Hourly, Non-Exempt | Full-Time | Location: Puyallup, WA

What You Bring
  • Credentials: Active MA, LPN, or RN license/certification in Washington State
