Job summary

Valley Health Partners Community Health Center in Allentown, PA seeks an Licensed Practical Nurse to provide patient care within the LPN scope and coordinate care across the continuum.

You will participate in network quality initiatives, maintain competence with clinical skills and EMR systems, and perform necessary administrative duties. BLS certification is required; 1–2 years of office-based clinic experience is preferred.

Qualifications

• Graduate of an accredited LPN program and valid state license.
• Experience in an office-based medical clinic preferred.
• BLS certification required; strong care coordination skills.

The Licensed Practical Nurse (LPN) provides care to designated patients within the Pennsylvania Licensed Practical Nurse Scope of Practice. Provides care coordination across the continuum. Participates in Network quality improvement initiatives. Demonstrates annual competence with clinical skills, clinical equipment, and electronic medical record system. Performs administrative functions as necessary.

Education:

• Graduate of an accredited Licensed Practical Nursing Program
• Bachelor’s degree in a health science, or a related field, preferred.

Experience:

• 1 – 2 years of experience working in an office based medical clinic, preferred.

Licensure:

• Basic Life Support (BLS)
