Job summary

Hospice of San Joaquin is seeking a skilled LVN to deliver compassionate nursing care within the hospice framework from Stockton. The role works with an interdisciplinary team to support patients and families in the San Joaquin and Stanislaus Counties.

Responsibilities include nursing visits, following RN Case Manager plans, documenting patient records, and coordinating with IDT. CPR training and strong communication skills are required.

Qualifications

  • A current license in California as a Licensed Vocational Nurse (LVN).
  • One year of recent clinical experience; home care or hospice preferred.
  • Ability to set priorities, organize work, and work independently.
  • Knowledge of medications, indications, dosage ranges, side effects, and toxicity.
  • Effective verbal and written communication skills.
  • CPR certification/training.

Responsibilities

  • Provide skilled nursing care per the IDT plan under supervision of the Chief Clinical Officer and RN Case Manager.
  • Perform nursing visits and assess patient and family needs.
  • Follow treatment plans established by the RN Case Manager.
  • Maintain up-to-date patient records.
  • Collaborate with IDT and Case Manager for actions and follow-up.
  • Support families at the time of patient death and coordinate with physician, mortuary, DME, and supplier.
  • Assist family in transitioning to bereavement with IDT support.

Position Purpose
  1. Provides skilled nursing care in compliance with the interdisciplinary team (IDT) plan of care, under the supervision of the Chief Clinical Officer and the direction of the RN Case Manager according to the policies and procedures of Hospice of San Joaquin.
  2. Using clinical skills, performs nursing visits; identifies the physical, emotional, physiological, spiritual, and practical needs of the patient and family.
  3. Follow the treatment plan established by the Registered Nurse/Case Manager.
  4. Follows scope of practice when providing nursing care.
  5. Makes recommendations to the Case Manager for action/follow‑up of patients.
  6. Mentains up‑to‑date patient records.
  7. On an ongoing basis, use clinical skills specific to the Hospice patient population to update the Case Manager and to give suggestions to the IDT for action and follow‑up by various team members.
  8. Follows universal precautions in delivering patient care.
  9. Provides support to families at the time of patient’s death with coordination of services with physician, mortuary, DME, and supplier.
  10. Assists family in making the transition from caregiving to bereavement, in coordination with IDT members.
  11. Demonstrates creativity in meeting patient/family needs in the home environment.
  12. Uses expert nursing judgment and functions as patient advocate, in collaboration with Case Manager, IDT, and primary physician.
Interdisciplinary Collaboration and Communication

Ensure continuity of care to patients/families by effectively collaborating and communicating with the IDT and the patient/family caregivers.

Job Qualifications
  1. A current license in the State of California as a Licensed Vocational Nurse.
  2. One year of recent clinical experience dealing with acute care patients preferred. Six months or more of home care or Hospice experience is preferred.
  3. Ability to set priorities, organize, and work independently.
  4. Knowledge of medications, indications, dosage ranges, side effects and potential toxicity.
  5. Skill in nursing practices with an understanding and knowledge of Hospice care and commitment to the philosophy and goals of the program.
  6. Ability to communicate effectively verbally and in writing.
  7. CPR training
