Job description

Position Summary

• Perform duties that address patient physical, psycho‑social and spiritual needs, including timely data gathering and ongoing monitoring.
• Incorporate cultural and ethnic factors into patient care, differentiating normal from abnormal findings and considering functional/developmental age for adult–geriatric patients.
• Implement and evaluate nursing aspects of the trans‑disciplinary care plan, collaborating with physicians and other disciplines.
• Provide individualized patient care plans and interventions, evaluate and document outcomes, and modify care plans as necessary.
• Administer routine pre‑operative nursing care and manage mechanical ventilation.
• Coordinate case‑management across the continuum, communicate effectively, and ensure patient safety regarding environment and equipment.
• Administer medications per policies, monitor outcomes, and report findings.
• Manage emergency situations, follow emergency procedures, and maintain chain of command for patient‑care issues.
• Maintain knowledge of tracheostomy care, ventilator patients, wound care, and long‑term patient support.
• Prescribe medications under Charge Nurse direction and demonstrate emergency care proficiency.
• Promote patient rights, privacy, and dignity; implement advance directives and involve families in decision‑making.
• Report suspected abuse or neglect as per policy.
• Identify patient/family educational needs, provide individualized teaching, and document effectiveness.
• Apply age‑specific care, including safety measures, normal values, skin integrity assessment, and behavioral considerations.
• Practice universal precautions.
