Job summary

The Orchards Health Center is seeking an LVN to provide skilled nursing care on a shift basis, including medication administration, treatments, documentation, and emergency response. You will support the RN supervisor and oversee CNA-directed care to ensure high-quality resident outcomes.

Ideal candidates hold a California LVN license, have at least one year of elderly care experience, and are familiar with Title 22 and OBRA regulations.

Qualifications

• Must hold a California LVN license in good standing.
• Experience delivering direct nursing care to elderly in skilled nursing settings preferred.
• Knowledge of resident care plans, documentation, and regulatory requirements (Title 22/OBRA).

Responsibilities

• Assist RN supervisor and oversee CNA direct care to maintain quality resident care.
• Assist with physician communication for changes in condition, orders, and medications.
• Notify families/surrogates of changes in condition and care plans; document communications.
• Respond quickly and professionally to medical emergencies; monitor for infection control.
