Job description

JOB SUMMARY

Under the supervision of the Administrative Nurse Manager, the Sr. Licensed Vocational Nurse utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio economic background and response to medical regimen and treatment. Additionally, they are required to learn and apply the Patient-Centered Medical Home Standards and Ten Building Blocks of Primary Care. They are responsible for providing nursing care to patients in the ambulatory setting. The Sr. LVN will work as an important member of the care team, working closely with the RN and the physicians to deliver all components of nursing care, individualized to meet the patient's physiological and psychosocial developmental level and needs. The Sr. LVN will be responsible for chart abstraction. The Sr. LVN consistently projects a professional and courteous demeanor, and shall comply with the Medical Center House and Telephone Standards. In addition, they will have clinical responsibility for administration of medications/vaccines, lab result review and monitoring, treatments and procedures pertinent to pediatric & adult primary care, such as ear irrigations, wound care and urethral catheterization, as well as patient education. The Sr. LVN must have flexibility to cover multiple UCSF medical center clinic sites.

This individual will utilize their good customer service and communication skills on a daily basis via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers. They will utilize computer skills in ApeX and other UCSF computerized programs. This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. They are required to adhere to the Telephone and House Standards as adopted by the Medical Center, as well as demonstrating UCSF's PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.

DUTIES & ESSENTIAL JOB FUNCTIONS
  1. Assessment
    1. Identifies the needs and responses of patients to medical regimen and treatment through observation of symptoms, emotional state, and cultural socioeconomic backgrounds.
    2. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patient to appropriate members of the health care team. Alerts a provider of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath.
    3. Learns how to respond in a code blue scenario and understands use of emergency equipment.
    4. Reports emergency or unsafe patient care conditions on the Incident Reporting System and to the management team.
  2. Planning
    1. Works cooperatively and participates with the health care team to discuss patient's alternatives to health care services and to develop short and long range goals to meet the patient's needs.

  3. Implementation/Evaluation
    1. Provides individualized nursing care which responds to the growth and developmental needs of patients and demonstrates an understanding of the range of treatment required by the patient.
    2. Demonstrates ability to provide appropriate care based on the needs of a specific individual, including that individual's age.
    3. Sets up and maintains cleanliness of exam rooms including periodic exam room disinfection and organization and removal of expired supplies.
    4. Performs intake process and engages in inreach and outreach to support practice quality initiatives and to support the population management.
    5. Provides for patient's physical and emotional privacy.
  1. Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments.
  2. Assists as needed with diagnostic and therapeutic procedures and treatments. This includes vaccines, injections, wound care and phlebotomy.
  3. Obtains specimens and performs Point of Care Testing as directed. Performs QA/QC on CLIA waived tests and reports any tests out of QA range to supervisor.
  4. Uses appliances and equipment correctly and maintains safe practice.
  5. Evaluates and documents patients' responses to nursing care in the electronic medical record, including pre/post procedures and treatments. Uses knowledge of cognitive, physical, emotional and chronological maturation process in evaluating care for all ages of patients.
  6. Administers and documents the administration of medication while maintaining current knowledge of pharmacology and competency in modes of administration.
  7. Documents telephone messages and chart notes in a thorough and grammatically correct manner.
  8. Responds appropriately in emergencies. Is knowledgeable about the contents of the code blue cart and/or the emergency kits, and can initiate and assist in CPR. Understands the procedures for medical emergencies, code blue and other emergency/disaster situations. Participates in safety drills.
  9. Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practice. Complies with activities mandated by TJC/Title 22/NCQA.
  10. Completes annual competency testing as specified by the individual practice.
  1. Organizes activities based on the needs of patients while maintaining the effective flow of patients through the practice. Adjusts clinical support activities to reflect changes in priorities during practice sessions.
  2. Maintains open communication and facilitates continuity of care for patients and/or family with members of the health care team through phone contact, verbal interaction.
  3. Maintains accurate, records and documentation which include patient complaints, nursing observations, interventions and care plan, nursing actions/teaching performed and patient response to the action.
  4. Obtains, records, and conveys information, clearly and concisely.
  5. Demonstrates courtesy, commonsense, and follow-through in all telephone/electronic communications, maintaining the Telephone Standards as established by the Medical Center.
  6. Completes all tasks within assigned InBasket to include cross-coverage of InBaskets of other teamlets and other practices across primary care.
  1. Fosters/maintains positive public relations (patients, family, visitors and staff). Uses effective communication skills with patients and staff; demonstrates proper telephone techniques and etiquette; shows sensitivity to differences of culture; demonstrates a positive and supportive manner in which patients perceive interactions as positive and supportive.
  2. Demonstrates ability to work as a team member with providers, supervisors, fellow nursing staff and clerical/administrative staff within the teamlet structure, with the practice team and in cross-organization relationships.
  3. Participates in daily huddles to support efficient and patient-centered care.
  4. Demonstrates Everyday PRIDE values and the AIDET fundamentals of communication in all patient care stakeholder interactions.

  2. Provides patients with the necessary information to make informed decisions utilizing available resources and making referrals appropriately. Informs patients/families about services available at UCSF Medical Center and in the community.
  3. Provides and reinforces patient health teaching by utilizing language which patient and family can understand.
  4. Utilizes and documents interpreting services as appropriate.
  5. Verifies patient/family understanding through return demonstration, either verbal, physical or written.
  6. Utilizes available educational resources. Participates in review and/or revision of educational materials.
  7. Participates in patient action planning and health coaching activities.
  1. Participates in Performance Improvement (PI) activities to include ongoing health coaching training, health care maintenance support and Patient-Centered Medical Home recognition.
  2. Makes recommendations regarding PI topics or problems, and assists in data collection.
  3. Reports and follows up on issues that affect environment and safety.
  4. Maximizes resources by seeking assistance from appropriate support, effectively using supplies, making suggestions for improved approaches, assists in cost effective practices.
  1. Attends classes, conferences, workshops and staff meetings applicable to practice needs. Shares information from programs attended.
  2. Attends and participates in staff meetings.
  3. Complies with work-related policies, including license renewal, annual PPD, CPR certification, use of benefit time, and dress code including identification policy.
  4. Completes all required local and Ambulatory competencies as well as online training.
  5. Refrains from using the internet or cellphone during normal work hours.
  6. Contributes to the orientation and learning experiences of other members of the health care team.
  7. Complies with departmental scheduling guidelines; i.e starts work at times scheduled, completes work in a timely manner.
  8. Adheres to PatientCentered Medical Home standards.
Required qualifications:
  • Minimum 3 years of experience as a Licensed Vocational Nurse in an ambulatory setting
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position, including demonstrated excellent communication skills, problem-solving ability, and high comfort level with learning new technology/systems.

  • Active American Heart Association BLS Certificate
