Job summary

Willow Creek Healthcare Center is seeking a compassionate Licensed Vocational Nurse (LVN) to join our nursing team on the PM shift. This full-time role focuses on delivering high-quality care during overnight hours while supporting resident safety and comfort.

Responsibilities include administering medications, monitoring residents, documenting in the PointClickCare EMR, and collaborating with CNAs and the interdisciplinary team. Competitive pay and full benefits are offered.

Qualifications

• Active California LVN license in good standing.
• Current CPR certification.
• SNF or long-term care experience preferred.
• Experience with EMR systems (PointClickCare preferred).
• Strong communication, organization, and time-management skills.

Responsibilities

• Provide direct nursing care to residents per care plans and physician orders.
• Administer medications and treatments accurately and on time.
• Monitor residents for changes in condition and report to charge nurse or physician.
• Document nursing care and observations in the EMR system.
• Maintain a clean, safe environment during overnight hours.
• Assist with admissions, discharges, and transfers as needed.
• Collaborate with CNAs and interdisciplinary team for resident-centered care.
• Follow infection control and confidentiality protocols.
