Job summary

HealthRIGHT 360, a nonprofit in Pasadena, seeks an LVN to join the outpatient team delivering nursing care within a compassionate, trauma-informed setting. You will participate in care planning, monitor clients, collaborate with physicians, and support medication administration and referrals in a DMH facility.

Work schedule is Monday–Friday, 9 am–5 pm, with a comprehensive benefits package and opportunities for professional development as part of our mission-driven team dedicated to health

Qualifications

• Completion of an accredited LVN program and CA LVN license.
• Current BLS and First Aid certifications.
• Valid California driver’s license and access to insured transportation.

Responsibilities

• Participate in the nursing care plan assessment, planning, implementation, and evaluation.
• Oversee ongoing monitoring and evaluation of clients in the Pasadena OP Adult DMH facility.
• Provide timely updates to the Physician for clients on withdrawal management, MAT, or with co-occurring mental disorders.
• Facilitate referrals to outside medical providers for medical clearance, TB tests or treatment.
• Assist with filling client prescriptions and coordinate with pharmacy for timely delivery.
• Monitor client care and take/record vital signs, assist with blood draws.
