Job summary

Senior Resource Group, LLC is seeking a Licensed Nurse for a full-time role in our Austin, TX community. The position involves delivering nursing services, mentoring Caregivers, and coordinating with healthcare providers to ensure quality resident care.

The role requires an active nursing license (LVN/LPN/RN), CPR/First Aid, and strong communication. A 1-year senior living background is preferred. This is a full-time daytime-oriented position with benefits and growth opportunities.

Responsibilities

  • Work as part of the Health Services Department team.

The Licensed Nurse is responsible for delivering nursing service duties, in accordance with Healthcare Provider orders while providing oversight to the resident care and Medication Management Program in accordance with healthcare provider orders, state regulations and SRG policies.This position does not have formal supervisory responsibilities; however, the Licensed Nurse will act as liaison with healthcare providers, families, pharmacies, and other healthcare agencies to maintain optimal care services for our residents. The Licensed Nurse will function as a mentor to Caregivers, Med Aides and/or Med Techs while keeping community leaders informed of routine and changing care needs of residents.

Essential Duties; Responsibilities:
  • Work as part of the team within the Health Services Department
  • Participates in and completes resident’ assessments to identify resident care needs.
  • Develops and updates resident service plans which include person centered approaches and interventions to determine resident care level.
  • Follows up on residents’ health conditions, and /or changes of condition with healthcare providers, residents and responsible parties.
  • Communicates relevant resident information to healthcare providers, alerts community staff and responsible parties of care concerns and service plan changes.
  • Performs nursing services as directed and in accordance with the Nurses Practice Act, and state regulations.
  • Develops and maintains effective relationships and communication with residents, families, responsible parties and other healthcare providers.
  • Collaborate with healthcare providers by reviewing weekly visit notes and ensuring continuity of care.
  • Assist with coordination of care for new move ins, residents on extended leave of absence and/or residents returning from alternate levels or receiving services from a new health care provider.
  • Completes documentation via Yardi EHR according to SRG policy and state regulations.
  • Initiate internal incident report and state reports as needed. Participates in internal incident investigations as needed.
  • Coordinates residents’ incidental medical, dental, vision, hearing, podiatry care and/or telehealth visits as needed.
  • Coordinate onsite lab diagnostic testing services and/or durable medical equipment as needed.
  • Completes Long Term Care insurance verification forms as needed.
  • Supports the Medication Management Program with a focus on the med room systems for medication delivery services, medication storage, and security.
  • Collaborate with preferred pharmacy partners.
  • May provide residents with ADL care and assistance with medication administration as needed.
  • Assist the Director of Assisted Living with daily staffing issues as needed.
  • Participate in daily crossover meetings and/or weekly Care Management meetings to communicate information regarding residents’ care needs to community team members.
  • Participates in Quality Assurance including investigations into unusual events, compliance audits, survey preparations.
  • Monitors the community’s infection control programs for compliance of care staff with safety programs.
  • Other duties as assigned.
Required Skills/Abilities/Education and Experience/Physical Demands:
  • Ability to work a flexible schedule, including weekends and holidays as needed.
  • Possess strong organizational and time-management skills and maintain consistent and regular attendance and have excellent oral and written communication skills.
  • Ability to comprehend, interpret, and apply the appropriate sections of applicable laws, guidelines, regulations, ordinances, and policies.
  • Proficient in applying nursing process within an Assisted Living and Enliven setting, displaying strong clinical evaluation skills.
  • Ability to work with Seniors and interact with cognitively impaired individuals.
  • Ability to be flexible, adapt and respond to change, make decisions in stressful situations, and prioritize tasks and projects.
  • Ability to analyze, solve and respond to problems or concerns.
  • Proficient with Microsoft Office, eMAR and other related software.
  • Must be a Licensed Vocational Nurse, Licensed Practical Nurse and/or Registered Nurse holding a valid nursing license in good standing.
  • Minimum of one (1) year of experience in a senior living environment preferred
  • First Aid and CPR certification required
  • Must pass a criminal background clearance before beginning work
  • While performing the duties of this job, the employee is regularly required to talk or listen. The employee is frequently required to sit, use hands to finger, handle, or feel, and reach with hands and arms. The employee must occasionally stand, walk, climb, or balance. The employee must occasionally lift and/or move up to 25 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
