Job summary

Sweetwater Care is seeking a licensed LVN/LPN to provide nursing care within scope of practice in our long-term care setting. You will deliver care per physician orders, follow center policies, and adhere to state, federal, and local guidelines to meet residents' needs.

You'll document changes in patient conditions, participate in rounds with physicians, and support narcotic counting, pharmacy orders, and safety initiatives. A compassionate team player with strong clinical skills is essential.

Qualifications

• Long Term Care experience required.
• Point click care charting experience preferred.
• Current BLS/CPR certification or willingness to acquire one.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
