Job summary

Enhabit Home Health & Hospice is seeking a Referral Coordination professional to manage patient referrals and act as a resource for referral sources. You will collaborate with branch and sales teams to ensure smooth coordination and transitions to services.

Knowledge of regulatory guidelines will be applied and demonstrated daily. The role requires a high school diploma, 2 years in healthcare, 1 year in home health or pediatrics, strong customer service, Medicare familiarity, and basic tech

Qualifications

• High school diploma or equivalent required.
• At least 2 years in health care and 1 year in home health or pediatrics preferred.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines that govern home health or hospice.
• Basic technology skills, including mobile devices.

Responsibilities

Coordinate patient referrals and serve as a resource for referral sources. Collaborate with the branch and sales teams to ensure adequate coordination and transition to services. Apply and demonstrate knowledge of regulatory guidelines and governing processes.

Qualifications

• Must possess a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• A graduate of an approved school of professional nursing and licensed in the state in which they currently practice is preferred; may be a registered nurse or licensed professional/vocational nurse.
• Experience with intake responsibilities and working with a multi-location company is preferred.

Requirements*

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions* For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license.
