Job summary

Assisted Home Health & Hospice is seeking a Licensed Vocational Nurse (LVN) for shift care in El Monte. You will work one-on-one with assigned patients and provide high-quality, safe home health nursing services.

Collaboration with the RN Case Manager and physicians ensures timely updates on patient conditions and needs. Requirements include graduation from an accredited practical nursing program and California LVN licensure.

Qualifications

• Graduate of accredited practical (vocational) nursing program.
• Licensed LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Responsibilities

• Work one-on-one with an assigned patient (full-time 40 hour day or night shifts are available).
• Ensure quality and safe delivery of home health nursing services.
• Communicate with RN Case Manager and physician on a regular basis regarding changes in the patient's condition and/or needs.
• Assist the RN in carrying out the plan of care.
