Job summary

Paycom - ATS is seeking an LPN to provide direct nursing care in a skilled nursing facility at Heritage at Brandon Place in Oklahoma City, OK. You will work under the supervision of a RN or Director of Nursing, administering medications and contributing to care plans while maintaining resident dignity.

Responsibilities include monitoring vital signs, documenting care, and collaborating with CNAs and the interdisciplinary team to ensure high-quality care and regulatory compliance.

Qualifications

• Current LPN/LVN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Experience in long-term care or skilled nursing preferred.
• Current CPR certification.

Responsibilities

• Provide direct nursing care to residents per care plans and physician orders.
• Administer medications and treatments as prescribed.
• Monitor residents’ vital signs, conditions, and responses to treatment.
• Report changes in resident condition to the RN or physician.
• Assist with the development and implementation of individualized care plans.

The LPN is responsible for administering medications, monitoring resident health, assisting with care plans, and ensuring high-quality care in accordance with facility policies and state regulations.

Key Responsibilities
