Job summary

Valor-Hospice seeks a Licensed Vocational Nurse (LVN) to provide bedside nursing care under the supervision of an RN in patients’ homes. You will coordinate with the Interdisciplinary Team, patients, families, and referring agencies to implement the plan of care and ensure continuity of high-quality care.

Requirements include graduation from an accredited LVN program and a current LVN license; a valid driver’s license is essential.

Qualifications

• Graduate of an accredited School of Vocational/Practical Nursing.
• Current driver’s license.
• Two year experience as a Licensed Vocational/Practical Nurse in a clinical care setting; Hospice experience preferred.

Responsibilities

• Under RN supervision, provide nursing care to patients in their place of residence.
• Coordinate care with Interdisciplinary Team, patient, family, and referring agency.
• Apply infection control and standard precautions to prevent contamination and transmission of disease.
• Ensure continuity of care through delivery of quality patient care and timely documentation.
• Communicate effectively with patient/family, team members, and other health professionals.

Job description

Primary function is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place ofresidence, coordinate care with the Interdisciplinary Group/Team, patient, family, and referring agency.Essential Functions:Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs asevidenced by documentation clinical record, team report, and evaluations.Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.Applies concepts of infection control and standard precautions in coordinating/performing patient care activities toprevent contamination and transmission of disease.Assures the continuity of care through delivery of quality patient care.Provides effective communication to the patient/family, team members, and other health care professionals asevidenced by clinical notes, case conferences, and communication notes and evaluations.Monitors assigned cases to ensure compliance with requirements of third party payor.Demonstrates commitment, professional growth, and competency.Promotes the Hospice philosophy and administrative policies to ensure quality of care.Education: Graduate of an accredited School of Vocational/Practical Nursing.Licensure: State license as a Licensed Vocational/Practical nurse, or a multi-state license issued by a member of the NationalNurse Licensure Compact (NLC).
