Job summary

Pilgrim Place In Claremont is seeking an on-call Neighborhood/Household Charge Nurse (LVN) for the NOC shift in Claremont, CA. The role requires graduation from an accredited nursing program and an active California LVN license, with preferred one year of skilled nursing or senior living experience.

You will administer meds, monitor residents, and support person-centered care in a mission-driven community. This on-call position is not eligible for benefits; hours are as needed.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current, active California Licensed Vocational Nurse (LVN) license.
• Minimum of one year of experience in a skilled nursing or senior living setting preferred.

Responsibilities

• Administer medications and treatments safely and according to plan of care.
• Observe residents’ physical and emotional condition and report changes to the Nurse Supervisor, Director of Nursing, or attending physician.
• Collaborate closely with the Nurse Leader to ensure regulatory compliance and best practices.
• Support a person-centered, resident-directed environment that fosters independence and choice.
• Serve as a mentor and role model for neighborhood/household staff, promoting professional nursing practices.
• Provide routine daily nursing care and services aligned with each resident’s plan of care and personal preferences.
• Advocate for residents’ needs and contribute to a safe, supportive, and homelike environment.
