Heritage Sierra Medical Group in California is seeking an Inpatient Case Manager (LVN) to assess and coordinate care for patients during acute episodes. You will work with an interprofessional team to plan treatment, document progress, and facilitate transfers within established guidelines.
The role requires an active LVN license in California and at least 1 year in case management. Strong communication, HIPAA compliance, and proficiency with Microsoft Office are essential.

The Inpatient Case Manager is responsible for the assessment, treatment planning, intervention, monitoring, evaluation and documentation on identified patients during an acute care episode. The pay range for this position at commencement of employment is expected to be between $33.00 and $36.00/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
