Job summary

Socket.dev LVN Case Manager in Los Angeles, under the supervision of a Registered Nurse, prioritizes patient needs and partners with discharge coordinators to plan safe discharge outcomes. You will collaborate with the interdisciplinary health care team to coordinate safe, cost-effective transitions of care.

You will perform utilization review and ensure clear, complete documentation in the medical record, advocate for patient self-determination, and support timely, appropriate discharge

Qualifications

• Two years of acute hospital case management experience or completion of Internal Case Management training program.
• Current Vocational Nurse licensure in the State of California.
• Computer/EMR proficiency and literacy.
• Knowledge of CMS, Medicare, Medi-Cal and Managed Care reimbursement.
• Familiarity with Joint Commission, CMS, CDPH requirements.
• Excellent written and verbal communication skills in English.
• Ability to establish and maintain effective working relationships across the organization.
• Ability to facilitate and lead interdisciplinary rounds.

Responsibilities

• Collaborates with patients and families to optimize outcomes and coordinate transfers; includes work with community resources, payers and health care stakeholders.
• Applies medical necessity criteria to determine level of care and length of stay; documents utilization review concurrently.
• Collaborates with patients/caregivers to set goals with physician plans and resources; documents in chart.
• Acts as liaison to medical staff to ensure continuity of hospital services.
• Participates in patient and family meetings and documents informed decision making.
