Job summary

Vasona Creek Healthcare Center seeks a highly organized LVN Case Manager to coordinate patient care, manage utilization reviews, and facilitate smooth transitions of care for residents. This full-time role requires strong communication with families, physicians, and multidisciplinary teams.

The ideal candidate has a California LVN license and at least 2 years of nursing experience, with preference for case management or utilization review in long-term care settings.

Qualifications

• Current California LVN license required.
• Minimum 2 years nursing experience; prior case management/utilization review in skilled nursing/long-term care preferred.
• Excellent clinical assessment, strong negotiation skills, insurance authorization skills, and Medicare/Medicaid knowledge.

Responsibilities

• Oversee discharge planning from admission to transition.
• Conduct utilization reviews and obtain authorizations with managed care/insurers.
• Act as primary liaison among patients, families, physicians, and care teams.
• Monitor resource utilization for cost-effective, compliant care.
• Maintain detailed care plans, case notes, and regulatory documentation.
