You will review records, coordinate preventive services, educate patients, and ensure accurate documentation across EMR systems while collaborating with providers and families to support ongoing, high-quality care.

Qualifications

• Graduate of a vocational/technical nursing program.
• Current LVN/LPN license in the state of practice.
• Knowledge of lab procedures, diagnostic testing, and EMR systems.
• Strong documentation, communication, and organizational skills.

Responsibilities

• Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols.
• Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support.
• Educate patients using evidence-based guidelines to promote engagement and health compliance.
• Maintain documentation accuracy, confidentiality, and compliance with all clinical and regulatory standards.
