Job summary

Lifespace Communities is seeking a Licensed Practical Nurse to join the team at Wesley Court in Abilene, Texas. The role focuses on coordinating interdisciplinary care, documenting resident status, and ensuring timely administration of treatments and medications while maintaining resident-centered service.

The LPN will collaborate with nursing staff to develop care plans, monitor census and levels of care, and communicate changes with families and physicians.

Qualifications

• Licensed Practical Nurse license required by state regulations.
• Graduate of an accredited nursing program.
• Documentation and timely nursing reports.
• Ability to coordinate admissions, discharges and transfers.

Responsibilities

• Review, plan, coordinate and evaluate resident's care with the interdisciplinary team.
• Document resident condition and nursing needs accurately and timely; report observations.
• Develop, direct, and monitor nursing assistant assignments and adjust based on census and care level.
• Execute treatments as necessary and document reactions to medications and treatments.
• Generate, prepare, administer, and chart medications.
• Initiate physician orders.
• Facilitate communication with families regarding changes in medications and resident condition.
• Establish protocol to ensure care plans are updated and reflect resident individuality.
• Coordinate admissions, discharges, and transfers to deliver quality customer service.
