Job summary

Dignity Health Woodland Memorial Hospital is seeking an LVN to provide direct patient care and support a comprehensive care team in our Woodland, CA facility.

Qualifications

• LVN experience within the last five years.
• Two years recent LVN experience in Home Health or Hospice.
• Licensed Vocational Nurse: CA license.
• Basic Life Support (American Heart Association).

Job Summary and Responsibilities

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.

Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

To be successful in this role, you will demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. Your compassionate demeanor, attention to detail, and ability to prioritize in a fast-paced environment are essential.

• Meets the discipline-specific productivity standard.
• Demonstrates competency in the care of patients during field observation by the clinical supervisor.
• Completes and submits documentation following established guidelines, as measured by defined evaluation processes (i.e. chart audits/QM reviews/etc.)
• Physician orders are written according to policy and reviewed/co-signed by RN.
• Documents follow-up of identified problems as clinically supervised by RN.
• Reviews and documents clinical progress toward goals.

Job Requirements

Required

• LVN Experience within the last five (5) years.
• Two (2) years recent experience as a LVN in a Home Health or Hospice Agency.
• Licensed Vocational Nurse: CA
• Basic Life Support from the American Heart Association.
• Driver's License: CA.
• Auto Insurance.
