Job summary

Midtown Oaks Post Acute in is seeking an LVN/LPN to lead shifts, supervise CNAs, and deliver high-quality patient care in a fast-paced skilled nursing environment.

You will mentor staff, ensure care plans are followed, and collaborate with physicians and the interdisciplinary team to maintain excellent resident outcomes.

Qualifications

• Active, unencumbered LVN/LPN license.
• Current CPR certification.
• Skilled nursing or long-term care experience preferred.
• Strong clinical judgment and leadership skills.
• Team-oriented mindset.
• Calm under pressure and solution-focused.
• Knowledge of PCC preferred.

Responsibilities

• Supervise and support CNAs to ensure high-quality care.
• Make rounds and ensure care plans are followed.
• Collaborate with physicians, therapy, and interdisciplinary team.
• Communicate clearly with families and residents.
• Create a calm, respectful, team-centered environment.
• Administer medications and treatments safely and accurately.
• Monitor resident condition and report changes.
• Complete accurate and timely documentation.
• Participate in admissions, transfers, and discharges.
• Respond confidently in urgent and changing situations.
• Support infection control and safety standards.
• Participate in quality assurance and care planning (MDS support).
• Assist with training and mentoring new staff.
• Help maintain a culture of accountability and compassion.

Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

$35-$37 per hour
Full time AM & On-Call
