Job summary

Mercy Medical Center Merced is seeking a Staff LVN to provide safe, therapeutic care using the nursing process, in collaboration with RNs and the treatment team. The role emphasizes independent practice within licensed scope and requires CA licensure and CPR/BLS at hire.

Located in Merced, CA, this position supports patients in home health/hospice settings with documented, timely care plans and coordination with case management to ensure consistent patient progress.

Qualifications

• 6 months to 1 year of Home Health/Hospice experience.
• Licensed Vocational Nurse (CA) licensure.
• American Heart Association Basic Life Support - CPR.
• Driver's License (CA) required.
• Auto Insurance required.
• Preferred: Acute care and skilled nursing facility experience.

Responsibilities

• Utilizes the nursing process to provide safe, therapeutic care for patients.
• Assessment planning in collaboration with RN.
• Documentation of the nursing process reflects a proficient level of clinical practice.
• Implements an established plan of care, follows frequency, works toward established goals and reports changes.
• Collaborates with RN Case Manager to assure effective, consistent management of clients and reports status changes promptly.
• Documents services rendered and client response; documentation is complete, accurate, timely and legible.

Job Summary and Responsibilities

The Staff LVN is an experienced practitioner who demonstrates in-depth knowledge of nursing, patient care, and the ability to practice independently within the confines of educational preparation, physician orders, and as assigned and supervised by a registered nurse.

• Utilizes the nursing process to provide safe, therapeutic care for patients. Assessment planning in collaboration with RN.
• Documentation of the nursing process reflects a proficient level of clinical practice.
• Implements an established plan of care, follows frequency, works toward established goals and reports any necessary changes.
• Collaborates, cooperates and coordinates with RN Case Manager/supervisor to assure effective, consistent management of clients. Works as a collaborative member of the treatment team and reports any changes in client status to case manager/supervisor in a timely manner.
• Documents services rendered and client response. Documentation is complete, accurate, timely, and legible.
