Job summary

Hollenbeck Palms is seeking a licensed LVN with supervisory experience to oversee daily nursing functions and provide direct patient care, under physician orders.

The role includes supervising CNAs, ensuring documentation accuracy, and maintaining compliance with infection control and patient rights while coordinating with RNs and physicians. The schedule follows a 4/2 rotation, and CPR/BLS certification is required.

Qualifications

Responsibilities

  • Provide direct patient care including medications, wound care, vital signs, and ADLs.
  • Maintain accurate medical records, care plans, and MARs.
  • Communicate with patients, families, and healthcare team to coordinate care.
  • Supervise CNAs and unlicensed staff to ensure quality patient care.
  • Ensure compliance with state/federal nursing regulations, infection control, and medication handling.
  • Collaborate with RNs, physicians, and other professionals on care plans.
  • Educate patients and families about conditions, treatments, and self-care.
  • Monitor patients for deterioration and intervene to prevent events or emergencies.

Job description

Position Purpose:

The primary purpose of job position is to supervise day-today nursing functions of an assigned area to include functioning as a Charge Nurse in providing care to residents as prescribed by the physician and in accordance with standards of nursing practices and regulations and ad many be directed by the Director of Nursing or Nursing Department Managers.

Schedule availability: 4/2 rotation schedule
Duties and Responsibilities:
  • Patient Care: responsible for providing direct patient care, including administering medication, dressing wounds, checking vital signs, and assisting with activities of daily living.
  • Documentation: required to maintain accurate and complete medical records, including progress notes, care plans, and medication administration records.
  • Communication: communicate with patients, family members, and healthcare team members to ensure that patients receive the appropriate care and treatment.
  • Supervision: supervise Certified Nursing Assistants (CNAs) and other unlicensed staff members to ensure that they are providing appropriate care to patients.
  • Compliance: comply with state and federal regulations related to nursing practice, including infection control protocols, medication management, and patient rights.
  • Collaboration: collaborate with Registered Nurses (RNs), physicians, and other healthcare professionals to develop and implement patient care plans.
  • Education: educate patients and their families about their conditions, treatments, and self-care techniques.
  • Safety: monitor patients for signs of deterioration and intervene as needed to prevent falls, injuries, and other adverse events to include emergency disasters.

Qualifications:

  1. Must be a licensed Vocational Nurse (LVN) in good standing and with a valid license by the State of California.
  2. Charge Nurse or supervisory experience preferred.
  3. Must clear criminal background with DOJ and FBI.
  4. Valid CPR/BLS Card
  5. Include educational, experience, and other pertinent requirements.
  6. Must be capable of performing the Essential Job Functions of this job with or without reasonable accommodations.
  7. Possess the ability to seek out new methods and principles and be willing to incorporate them into existing activity programs.
  8. Have a sincere desire to work with the aged.
  9. Must meet all local health regulations, and pass post-employment physical exam, if require.
  10. Must be able to relate positively and favorably to residents to residents and to work cooperatively with other associates.
  11. Ability to read and write in a legible and understandable manner.
Physical Requirements of Position
  1. Must be in good general health and demonstrate emotional stability. Must be able to move intermittently throughout the work day
  2. Possess the ability to cope with mental and emotional stress of the position, as well as ability to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility
  3. Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.

Physical and Sensory Requirements (with or without the aid of Mechanical devices): Walking ability, able to distinguish smells; ability to communicate with residents and personnel, ability to read, write and ability to understand and instruct nursing services personnel concerning training and education.
