Glendale Memorial Hospital and Health Center is seeking a Licensed Vocational Nurse (LVN) to provide direct patient care in a compassionate team setting. You will assess patients, administer medications, document care, and collaborate with RNs and physicians to implement individualized care plans.

Responsibilities

  • Implements and documents interventions based on nursing and medical plan of care. Acts as a patient advocate. Guides team members for assigned patient care interventions.
  • Uses equipment and supplies per policies and protocols; logs routine QC activities per hospital policy.
  • Ensures patient safety per JCAHO standards; avoids unapproved abbreviations; writes legibly.
  • Administers medications within LVN scope; double checks meds and documents in medical record; notes lactation status, allergies, height and weight.
  • Demonstrates AIDET principles; assesses patient/family needs and provides individualized care.
  • Attends and participates in staff meetings; maintains confidentiality; supports risk management and performance improvement.

Job Summary and Responsibilities

As our Licensed Vocational Nurse (LVN), you will provide vital direct patient care, significantly contributing to patient well-being and recovery as an integral part of our compassionate healthcare team.

Every day, you will assess patient conditions, administer medications, perform prescribed treatments, and meticulously document care. You will collaborate with RNs and physicians to implement individualized care plans, ensuring a seamless and high-quality patient experience.

To be successful in this role, you will demonstrate exceptional clinical skills, a strong commitment to patient safety, and excellent communication. Your compassionate demeanor, attention to detail, and ability to prioritize in a fast-paced environment are essential.

  • Implements and documents interventions based on nursing and medical plan of care. Acts as a patient advocate. Provides guidance to team members as needed for assigned patient care interventions. Functions as a highly valued member of the Women's Center Team.
  • Appropriately utilizes equipment and supplies (including following established policies, procedures, and protocols) in the care of the patient. Supervise routine QC activities log- per hospital laboratory policy.
  • PATIENT SAFETY: Provides for patient safety according to JCAHO standards. Restricts use of unapproved abbreviations. Writes legibly.
  • MEDICATION SAFETY: Administer medications within LVN scopes and adheres to medication safety standards per hospital policy. Double checks medications as required and documents in medical record. Identifies lactation status, allergies, height and weight on all patients.
  • PATIENT SATISFACTION: Demonstrates AIDET Principles. Assesses patient/family personal needs and provides Individualized Care.
  • WORK HABITS: Attends at least 50% of staff meetings each year and reviews 100% of staff meeting. Observes hospital policy regarding smoking, telephone usage, and other rules governing conduct while at work. Maintain confidentiality of patient information. Participates in Performance Improvement activities as appropriate. Identifies and communicates risk management issues appropriately.

Required

  • Vocational School Grad Education required to obtain LVN license
  • Licensed Vocational Nurse: CA
  • Basic Life Support - CPR, upon hire
Preferred
  • 1 years experience in an outpatient OB/GYN setting
  • Bilingual (English/Spanish)
  • CPSP Educator
