Job summary

The Transplant Coordinator at UCLA Health oversees referral relationships and patient flow for lung transplant services in the greater Los Angeles area. You will coordinate evaluations, manage care from referral through follow-up, and liaise with physicians, payers, and external practices to optimize access to advanced lung therapies.

Ideal candidates have CA RN licensure with BSN/MSN and transplant/ICU experience; CCTC is required for TC II.

Qualifications

• California RN license with BSN/MSN and 5 years RN experience in critical care/transplant (TC I).
• TC II requires CCTC certification and 5 years RN experience in transplant/critical care.
• Ability to manage transplant patient care from evaluation to follow-up.
• Strong communication to work with diverse backgrounds.

• Ability to teach transplantation and medical management; capable of data entry and high-volume workload.

Primary Duties and Responsibilities

The Transplant Coordinator’s responsibilities are related to maintaining and enhancing referral relationships with physicians, payers and major employers. Responsible for servicing clients located in the greater Los Angeles area. Facilitates access to UCLA Health System for advanced lung disease therapies. Clinical Liaison for referred patients, including coordinating/scheduling appointments and transplant evaluations both inpatient and outpatient. Act as Liaison for the referring physicians and clinical practices outside of UCLA main campus; assist in seeing patients, obtain detailed medical history and results of medical investigation and procedures. Assess patient services satisfaction through feedback from patient referral sources. Participate in clinical research projects and screening potential patients that may qualify for clinical trials. Assist with IRB applications. Must have knowledge and expertise in lung transplantation, and advanced lung disease care. These include but are not limited to clinical case management of the transplant recipient involving all phases of the transplant process, from initial referral through outpatient follow up.

Salary range:

TC1: $181,071.365-$243,419.04 (annually)

TC2: $ 218,175.12 - $ 260,749.44 (annually)

TC2: must have CCTC

Job Qualifications

1. TC I:Current California Registered Nurse license, graduation from an accredited nursing program and a BSN or Master’s degree with 5 years of relevant RN experience with a combination of critical care and transplant coordinator experience (required).

2. TC II: Current California Registered Nurse license, graduation from an accredited nursing program and a BSN or Master’s degree with a CCTC required and 5 years of relevant RN experience with a combination of critical care and transplant experience (required).

3. An ability to case manage all aspects of a transplant patient’s medical care from initial evaluation to post transplant follow-up; apply a broad understanding and knowledge of organ transplantation, including immunology, immunosuppression, rejection and other complications of transplantation, drug toxicity, and graft failure.(required)

4. Ability to communicate effectively with persons of various cultural backgrounds. Good verbal and written communication skills.

5. Ability to interface effectively with patients and their families.

6. Excellent organizational skills, proficiency in meeting deadlines, ability to work independently and prioritize workload. Ability to manage numerous concurrent tasks in a high stress environment and deal with change.

7. Ability to effectively coordinate and assess patient care and act as a liaison with nursing staff, outside agencies, team physicians, referringphysicians, patients and families. (required)

8. Ability to teach about transplantation and medical management issues. (required).

9. Ability to walk/navigate throughout the Medical Center campus, including surrounding buildings.Frequent sitting and standing. Ability to lift packages and patient charts up to 25 pounds for distances to 10 feet. Strong typing skill necessary for large volume data entry into transplant database and Care Connect.Ability to handle volume, high stress clinical environment (required)

10. Knowledge and experience in lung transplantation and advanced lung disease (required).
