Job description

Immediate need for a talented LTSS Service Care Manager. This is a 6+ months contract opportunity with long-term potential, located in Orlando, FL (Hybrid). Please review the job description below and contact me ASAP if interested.

Pay Range: $23 - $25/hour. Employee benefits include health insurance (medical, dental, vision), 401(k), and paid sick leave (depending on work location).

Key Responsibilities:
  • Managing a caseload for healthcare members with long-term care needs.
  • Monthly and quarterly member contact, involving 80% travel. Remote role. Driver’s license required.
  • Conducting member assessments and documenting notes.
  • Completing assessments with members, caregivers, or providers to gather information for care plan development.
  • Monitoring service delivery and following up via in-person visits and phone contacts.
  • Authorizing and coordinating referrals for services.
  • Ensuring provider services are delivered without gaps and identifying deficiencies in care plans.
  • Assisting in developing informal or voluntary services to support member care plans. Collaborating with discharge planners, physicians, and other stakeholders to ensure proper discharge and care plans, and coordinating acute and long-term care services.
  • Helping members file and resolve complaints and appeals.
Key Requirements and Technology Experience:
  • Experience with chronic disease management
  • Experience in long-term care
  • 2+ years of care management experience
  • Experience with electronic medical records
  • Bilingual in Spanish; proficiency in Microsoft Office
  • Bachelor’s degree and 2-4 years of relevant experience
