This position is field-based with in-person visits to member homes or facilities required up to 75% of the time. 25% of the role is remote/home-based. Candidates should reside in Fort Myers, FL to be considered for the role.

Position Purpose: Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans for long-term care members and educates members and their families/caregivers on available services and benefits.

• Evaluates member needs, available resources, and recommends or facilitates the best outcome plan.
• Assists with developing ongoing long-term care plans and identifies providers, specialists, and community resources.
• Coordinates between members, families, caregivers, and care providers to ensure service accessibility.
• Provides resource support for needs such as employment, housing, independent living, and justice issues.
• Monitors care plans, member status, outcomes, and provides recommendations.
• Interacts with healthcare providers and partners to meet member needs.
• Documents and maintains member information to ensure compliance with regulations.
• May perform home/site visits to assess needs and collaborate with providers.
• Provides education on procedures, healthcare options, and benefits.
• Offers feedback to leadership for quality improvement.
• Performs other duties as assigned and complies with policies.

Education/Experience: Bachelor's degree and 2-4 years of related experience or equivalent knowledge and skills.

Additional Information:

• Candidates should reside in Fort Myers, FL.
• Fluent bilingual in English/Spanish highly preferred.

Pay Range: $26.50 - $47.59 per hour
