Job summary

Highmark Inc. is seeking a community-based Care Coordinator to serve as the primary contact for members’ care needs across service delivery systems, traveling within Delaware.

The role involves direct home visits, nursing facility interactions, and collaboration with an integrated care team to optimize outcomes. Responsibilities include performing needs assessments, developing individualized care plans, coordinating LTSS and Medicaid benefits, and guiding members toward appropriate community

Qualifications

• Bachelor's degree in Social Work or related field.
• Master’s degree in Social Work or related field preferred.
• Qualified candidates with nursing licensure or eNLC are welcome.

Responsibilities

• Conduct regular in-home and facility visits to assess needs and coordinate care.
• Develop and monitor individualized care plans with member and caregiver input.
• Coordinate LTSS, Medicaid, and DSNP benefits across care settings.
• Educate members and caregivers on benefits, resources, and plan of care.
• Ensure privacy compliance and accurate case documentation in EHR.
