Avenues Recovery Center in South Bend, IN is seeking a Licensed Practical Nurse (day shift) for our detox and residential treatment facility. You will perform comprehensive assessments, manage nursing duties, dispense medications, and coordinate with the medical team to ensure high-quality patient care.

Ideal candidates have a valid LPN license in Indiana (or compact), at least 1 year in a treatment setting, and strong communication skills to support a multidisciplinary team and accurate

Qualifications

• Must hold current, valid nursing license in-state or compact license with multistate privileges.
• 1 year of experience in treatment/healthcare settings preferred.
• Strong critical thinking and both verbal and written communication skills.

Responsibilities

• Complete comprehensive medical assessment for each client upon admission.
• Perform all nursing shift duties (screening, vitals, scheduling and labs, chart updates).
• Address pharmacy needs and prescription authorizations; dispense meds and maintain logs.
• Collaborate with medical, clinical and UR teams to coordinate care.
• Ensure meticulous medical documentation, chart reviews, and medication logging.

What You'll Do

• Complete comprehensive medical assessment for each client upon admission
• Perform all nursing shift duties (screening, vitals, scheduling appts and labs, updating charts...)
• Address pharmacy needs and prescription authorizations; dispense meds and maintain med logs
• Collaborate with medical, clinical and UR teams to ensure seamless coordination of care
• Ensure impeccable medical documentation, chart review, and medication logging for each client

What We're Looking For

• Current, valid licensure to practice nursing in-state, or compact license with multistate privileges
• 1 year experience in treatment industry preferred
• Strong critical thinking, oral and written communication skills
• Excellent collaborative, multitasking and time management skills
