Job summary

Avenues Recovery Center in South Bend, IN is seeking a Licensed Practical Nurse (LPN) for day shifts at our Detox and Residential facility. You will conduct admissions assessments, monitor vitals, dispense medications, and help coordinate care with the medical and clinical teams.

The role requires current Indiana nursing licensure (or compact license) and strong communication and organizational skills. Prior experience in treatment is preferred.

Qualifications

• Current, valid nursing licensure in-state or compact license with multistate privileges.
• 1 year experience in treatment industry preferred.
• Strong critical thinking and communication skills.
• Excellent collaboration, multitasking and time management skills.

Responsibilities

• Complete comprehensive medical assessment for each client on admission.
• Perform nursing shift duties (screening, vitals, scheduling appts and labs, updating charts).
• Address pharmacy needs and prescription authorizations; dispense meds and maintain med logs.
• Collaborate with medical, clinical and UR teams to ensure seamless coordination of care.
• Ensure impeccable medical documentation, chart review, and medication logging for each client.

What You'll Do

What We're Looking For
