Responsibilities

• Manage the day-to-day healthcare operations of the community to ensure residents' healthcare needs are met.
• Ensure residents are treated with respect and dignity and receive quality care as needs change.
• Maintain consistent quality resident care and service across all shifts; may assume Caregiver or Med Tech duties to meet staffing needs.
• Supervise licensed nursing staff, medication technicians/CMAs, and caregivers; provide ongoing training and education.
• Oversee continuity of medication training, pharmacy management, and medication supervision/administration; conduct audits.
• Share on-call duties as required.
• Education required to obtain NY nursing license (LPN/LVN or RN) and 1-2 years relevant experience.
• LPN or LVN license.

LPN Wellness Coordinator (Finance)

Brookdale Niskayuna is seeking an LPN Wellness Coordinator to assist in overseeing the clinical department.

As a Wellness Coordinator you will help manage the day-to-day clinical services of the community to verify that resident's activities of daily living (ADLs) and healthcare needs are met. Verify residents are treated with respect and dignity and verify residents are receiving quality care as their needs change.

Required skills and qualifications:

• * Must have at least 1-2yrs nursing experience
• * Must have management/supervisory experience in overseeing a clinical care team
• * Experience in senior living preferred
• * You have provided training and education to caregivers/nurses on an ongoing basis to include classroom in-services and situation-specific training
• * You have participated in on-call duties

Base pay in range will be determined by applicant's skills and experience. Temporary associates are not benefits eligible but may participate in the company's 401(k) program.

• Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met.
• Ensures residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change.
• Ensures consistent quality resident care and service delivery throughout all shifts; this may require assuming the role and responsibilities of a Caregiver or Med Tech/Aide in order to meet the residents' needs and staffing requirements.
• Assists in the supervision of community-based licensed nursing staff, medication technicians/CMA's, and Caregivers and provides training and education to Caregivers on an ongoing basis to include classroom in-services and situation-specific training. In addition, they will also manage the associates schedules.
• Oversees and manages the continuity and consistency of medication training, pharmacy management and medication supervision and/or administration in the community. Conducts periodic Care Associate medication skills inventory checks, and periodic medication audits per Brookdale Guidelines.
• Shares on call duties as required.
• Education as required to obtain state nursing license (LPN/LVN or RN) and a minimum of 1-2 years relevant experience.
• LPN or LVN license.
