Job summary

Heritage Park in Fort Wayne, IN is seeking a Licensed Practical Nurse for a weekend night shift (6p-6a) on Saturdays and Sundays. You will provide direct nursing care, assess residents, document changes, and collaborate with the care team to ensure high-quality, patient-centered care.

Current Indiana LPN license required; strong geriatric care passion and excellent communication are essential. ASC emphasizes CARE values: Compassion, Accountability, Relationships and Excellence.

As a Licensed Practical Nurse (LPN), you will have the opportunity to develop meaningful relationships by learning about the life stories of our residents and staff.
