Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) Visit Nurse to provide in-home patient care under a prescribed plan of care. You will monitor progress, perform assessments, and collaborate with other providers to adjust care as patients' needs evolve.

The role requires a current LPN license, CPR/BLS/First Aid certification, reliable transportation, and at least 2 years of home health experience. Flexible scheduling and 1:1 patient care are offered.

Qualifications

• Graduate of a practical nursing program and current LPN/LVN license in the practicing state.
• Current CPR/BLS/First Aid certification.
• Reliable transportation to care sites and/or work locations.
• Two years of experience as an LPN in home health or similar setting.

Responsibilities

• Provide in-home direct patient care per the plan of care and nursing scope.
• Complete skilled assessments and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to adjust care as needed.
• Document all clinical activities in accordance with agency protocol and Medicare guidelines.
• Provide education to patients and family members involved in care.
• Help reduce readmissions by promoting preventative care and continuity of care.

As a Licensed Practical Nurse (LPN) Visit Nurse you will:

• Provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery
• Complete skilled assessments and provide exceptional patient-centered care in the homeEnsure an accurate evaluation and implement a comprehensive care plan that addresses patient needs
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve
• Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
• Provide education to patients and family members involved in patient's care.
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care

To qualify for a Licensed Practical Nurse (LPN) position with us, you will need:

• Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as an LPN/LVN in the state(s) in which he/she practices and have at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Two (2) years of experience practicing as a Licensed Practical Nurse (LPN in a home health or similar setting
