Responsibilities

• Monitors electronic messaging related to patient and provider requests within scope and responds promptly.
• Determines needs and initiates follow-up telephone contact within scope of practice.
• Documents patient data according to established guidelines and policies.
• Provides health education and counseling to patients and families and evaluates effectiveness of the education process.
• Advocates for the patient and participates in quality improvement activities.
• Adheres to confidentiality, privacy, and Kaiser Permanente policies.
• Performs other department-specific duties within scope of practice.
