You will develop care plans, coordinate admissions and discharges, and participate in QA activities. Strong leadership, clinical expertise, and clear communication are essential for success.

Qualifications

• Must possess a current, unencumbered, active LPN license in Ohio
• Must demonstrate the knowledge and skills necessary to provide care appropriate to age-related needs of residents
• Must be able to read, write, speak, and understand English
• Must be a supportive team member and contribute to teamwork
• Must possess the ability to make independent decisions when warranted
• Must be knowledgeable of nursing practices, procedures, and applicable laws/regulations for nursing facilities
• Must possess leadership and supervisory ability and work harmoniously with staff
• Must be willing to adopt new methods and principles into nursing practices

Responsibilities

• Provide direct nursing care to residents and supervise day-to-day nursing activities performed by STNAs
• Ensure all nursing personnel comply with applicable standards, guidelines, and regulations
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Coordinate and monitor resident care, including admission, transfer, and discharge
• Ensure nursing staff follow policies and report deviations to the Nurse Supervisor and Unit Manager
• Participate in quality assurance programs, surveys, and inspections
• Collaborate with dietary, social services, and activities for holistic resident care
• Provide education and training to nursing personnel as needed
• Conduct performance evaluations and provide feedback for staff development
• Maintain accurate and detailed resident care records and documentation
