Northgate Health Care Facility in Tonawanda, NY seeks an experienced LPN Unit Coordinator of Subacute to lead daily clinical operations on a subacute unit and ensure high-quality resident care. You will supervise staff, coordinate with the care team, and uphold regulatory compliance in a CMS 4-Star facility.

The role offers full-time schedule, day shift, and a competitive $30–$32/hour plus a sign-on bonus up to $3,000.

Qualifications

• Active NYS LPN license required.
• 1 year LPN experience in long-term care preferred.
• Supervisory experience a plus.

Responsibilities

• Oversee clinical care and daily operations of the subacute unit.
• Supervise nursing staff and support staff; provide leadership.
• Monitor resident conditions, update care plans as needed.
• Coordinate with physicians, nurse practitioners, residents and families.
• Review labs, orders, incidents, and changes in condition.
• Participate in care plan meetings and interdisciplinary discussions.
• Assist with admissions, discharges, and transfers.
• Ensure accurate documentation and resident records.

In this role, you'll oversee the daily clinical operations of an assigned nursing unit, ensuring high-quality resident care, regulatory compliance, and effective coordination among residents, families, providers, and interdisciplinary team members. RESPONSIBILITIES:

• Oversee the clinical care and daily operations of the subacute unit
• Supervise nursing staff, nursing assistants, and unit support staff while providing leadership and guidance
• Monitor resident conditions, review assessments, and ensure care plans are updated and implemented appropriately
• Coordinate communication with physicians, nurse practitioners, physician assistants, residents, and families
• Review laboratory results, provider orders, incident reports, and changes in resident condition
• Participate in care plan meetings, QAPI activities, family meetings, and interdisciplinary team discussions
• Assist with admissions, discharges, transfers, and coordination of outside appointments
• Ensure unit supplies, documentation, and resident records are accurate and current
• Support staff education, orientation, infection prevention efforts, and quality improvement initiatives
• Address resident, family, and staff concerns while promoting a positive care environment
• Perform additional duties as assigned

REQUIREMENTS:

• High School Diploma or equivalent required; Associate's Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Minimum 1 year of LPN experience in a long-term care setting required
• 3-5 years of long-term care experience with supervisory experience preferred
• Strong knowledge of resident care planning, clinical documentation, and long-term care regulations
• Leadership experience with the ability to supervise, coach, and mentor staff
• Excellent communication, organizational, and problem-solving skills
• Ability to coordinate care across multiple disciplines and manage competing priorities
• Commitment to providing compassionate, resident-centered care
