Job summary

Cleveland Clinic Lutheran Hospital is hiring an LPN to support the 4D Medical/Surgical and Telemetry Unit, delivering high-quality patient care across cardiac, respiratory, GI, and post-operative needs while integrating into the Cleveland Clinic’s renowned care network.

You will administer prescribed medications, assist with ADLs, perform accurate assessments and data collection, collaborate with RNs on care plans, educate patients and families, and participate in ongoing competency training,

Qualifications

  • Graduate of an approved school of Practical Nursing.
  • One year of clinical experience within the previous three years or graduate within the previous six months of hire.
  • Licensed Practical Nurse (LPN) licensure and current registration in the State of Ohio.
  • For specific areas, must be certified in medication administration and IV therapy as approved by the Ohio Board of Nursing.
  • Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross certification within one year of hire.

Responsibilities

  • Perform accurate and complete admission, ongoing assessments and discharges within the defined time frames; assesses and documents patients' pain levels utilizing appropriate protocols.
  • Collaborate with the RN on the formulation, implementation, evaluation and revision of patients' care plans.
  • Notify RN of any pertinent physical, psychosocial, and emotional findings.
  • Provide direct care to assigned patients and complete hourly rounding and the rounding log as assigned.
  • Delegate and evaluate the care administered by non-licensed personnel.
  • Administer prescribed medications and treatments in accordance with licensure and approved nursing protocols.
  • Apply knowledge of usual dosage, indications and side effects of medications administered.
  • Utilize the 5 rights of medication administration.
  • Ensure that all medications are secured at all times.
  • Utilize appropriate reporting mechanism for any medication errors, near misses, etc.
  • Assess the educational needs of the patient and significant others considering cultural issues and other special concerns.
  • Collaborate with the RN assigned to the patient on educational needs of the patient.
  • Provide basic teaching and educational material to the patient/family on diagnosis, tests and procedures, safe and proper use of equipment, reason for and safe use of medications, food/drug interactions, injury prevention, self-care and nutritional needs.
  • Review discharge instructions as previously presented by the Physician/RN; documents teaching.
  • Complete appropriate annual competency requirements and training as required.

The 4D Medical/Surgical and Telemetry Unit provides cardiac, respiratory, gastrointestinal, genitourinary, neurological, infectious disease, substance abuse, behavioral health, pre-operative and post-operative care. Known for exceeding expectations and high-quality care, this family of caregivers always puts patients first. As an LPN on this team, you will assist fellow nursing staff in daily department activities, such as medication administration, assistance with ADLs and collecting patient data. A caregiver in this role works nights from 11:00 p.m. - 7:30 a.m.

A caregiver who excels in this role will:

Minimum qualifications for the ideal future caregiver include:
  • Graduate of an approved school of Practical Nursing
  • One year of clinical experience within the previous three years
  • OR must have graduated within the previous six months of hire
  • Licensed Practical Nurse (LPN) licensure and current registration in the State of Ohio
  • For specific areas, must be certified in medication administration and IV therapy as approved by the Ohio Board of Nursing
  • Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross certification within one year of hire
Physical Requirements:
  • Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination.
  • Requires standing and walking for extended periods of time.
  • Requires corrected vision and hearing to normal range.
  • Requires working under stressful conditions and irregular hours.
  • Exposure to communicable diseases and/or body fluids.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Physical Demand requirements are in excess of those for Light Work
Personal Protective Equipment:
  • Follows standard precautions using personal protective equipment as required
