JOB SUMMARY

Participates in coordination, planning, implementation, and evaluation of care provided by the Patient Care teams under the direction of the provider team leader but within the objectives, standards, and policies of the center and within the parameters of his/her preparation. The LPN Team Lead assumes leadership in the absence of the Director of Nursing, Deputy Director of Nursing, or the RN.

ESSENTIAL FUNCTIONS
  • Participates in Care team huddles- initiates pre-visit planning workflow to ensure care completion prior to visit whenever possible
  • Exhibits excellent customer service at all times. Maintains a professional appearance and attitude and always displays to internal and external customers.
  • Monitors and stays aware of the customer's wait time.
  • Perform Patient assessment for non-appointment patients (walk-ins) and all phone calls. Prioritize walk-ins according to the urgency of the patient’s condition.
  • Assess vital signs (temperature, pulse, respiration, blood pressure, weight, height, etc.).
  • Place patients in examination rooms with proper drapes and privacy. Prepare patients for exam procedures.
  • Keep exam rooms clean, organized, and stocked.
  • Comprehensive assessment, including documentation of symptoms, chief complaints, medication reconciliation, and allergy history.
  • Initiate standing orders related to established guidelines adopted by FPHC.
  • Administer medications as authorized and prescribed by the physician.
  • Documents correctly all treatments, medications, lab results, phone messages, referrals, etc.
  • Assist physicians with treatments, examinations, injections, x-rays, EKGs, heart monitors, assessments, etc.
  • Assist and respond to emergencies.
  • Monitor supply levels and reorder when needed.
  • Ensures coordination and tracking of all labs, referrals, diagnostic tests, and any abnormal lab findings as required.
  • Maintains crash cart, O 2 tanks, narcotic box, refrigerator, medical supply room, medication closet and IV cart. Responsible for completing and reporting findings on a log sheet to the supervisor at the end of the reporting period.
  • Involving the patients in activities to improve their health (patient engagement). Use skills for patient assessment and implement patient teaching/education/self-management.
  • Participates in PCMH Care team quality improvement activities, including care team performance on required clinical, operational, and financial measures.
  • Performs venipuncture and basic lab work as needed.
  • Ensures adequate staffing levels/coverage for the clinical unit and the Director of Nursing.
  • Provide day-to-day supervision for the patient-care unit nursing staff.
  • Prepares work and lunch schedules for the patient-care unit nursing staff
  • Complies and tracks data for department reports as required.
  • Performs other duties as assigned
PEDIATRICS

Give immunizations, PKUs, Medicaid screenings, vision/hearing exams, TB skin tests, lab work (when necessary), strep tests, and obtain urine analysis. Order immunizations through the VFC Program and submit data to the area coordinator as needed.

OB/GYN

Assists with Colposcopy and Cryosurgery, Norplant insertion, OB intake, Family Planning, TB skin tests, and lab work (when necessary)

ADDITIONAL RESPONSIBILITIES
  • Maintains current Alabama Nursing License and CPR certification. Remain abreast of current employee health practices and guidelines through continuing education classes, journals, and professional organizations.
  • Attends continuing education related to infection control practices, monitoring, and surveillance of infectious disease processes within an organization.
  • Maintains a professional approach with confidentiality.
  • Observe and support Center policy and assist with staff education and readiness preparation for JCAHO site visits.
  • Is prompt and efficient with minimal absences.

PERFORMANCE STANDARDS

The Director of Nursing and/or designee will evaluate performance annually, and a satisfactory overall rating is considered a minimum acceptable level of performance.

HEALTH AND SAFETY REQUIREMENTS
  • Must adhere to safety regulations, personnel policies, and procedures.
  • Safety and Hazard Communication training update is required annually.
  • Knowledgeable of OSHA and CLIA requirements to assure compliance.
  • Maintain yearly health maintenance records by obtaining TB skin testing and completing the hepatitis series.
PRIVACY RULE COMPLIANCE REQUIREMENTS
  • Adheres to Health Insurance Portability and Accountability Act (HIPAA Privacy Rule) policies and procedures.
  • Must complete Privacy Rule compliance training annually or as revisions are made to the policies and procedures.
  • Must comply with Privacy Rule guidelines by learning to protect FPHC patient’s medical privacy.
  • Must comply with Privacy Rule guidelines by appropriately maintaining our patient information in compliance with national standards.
  • Must comply with Privacy Rule guidelines by providing appropriate security of FPHC patient records.
Qualifications
QUALIFICATIONS

Diploma in Nursing, graduate from an accredited nursing school. Current license to work in the State of Alabama. Certification in IV and/or experience in infusions is required. One year of clinical work experience is preferred.

Demonstrated clinical competence. Assumes responsibility for own continued personal and professional growth. Demonstrate appropriate manner, conduct, grooming and sound judgment. Demonstrated ability to work cooperatively and communicate effectively. Good health and consistent attendance. Membership and participation in the AL Board of Nursing Association are encouraged.

CERTIFICATION, LICENSES, REGISTRATIONS

LPN license issued by the State of Alabama; CPR Certification; Valid driver's license.

PHYSICAL/MENTAL DEMANDS

Requires working under stressful conditions and irregular hours. Must be able to express ideas clearly and concisely and exercise balanced judgment in evaluating situations and making decisions. Work requires sitting, standing, stooping, bending, driving, walking, moving, examining objects at high and low reach, and reading. Must be able to lift and move patients as needed.
