Qualifications

• Active Licensed Practical Nurse (LPN) license in Ohio.
• Strong clinical, communication, and critical thinking skills.

Responsibilities

• Provide direct nursing care to residents in a long-term care and skilled nursing setting.
• Administer medications and treatments as prescribed by physicians.

• Collaborate with RNs, STNAs, and other healthcare professionals to ensure quality, patient-centered care.
• Maintain accurate, detailed medical records and documentation in compliance with regulations.
• Support rehabilitation and restorative nursing care programs.
• Participate in care planning and assist with care coordination.
