Signature HealthCARE of Roanoke Rapids is hiring a licensed practical nurse (LPN/LVN) to provide direct nursing care to residents in a 108-bed facility in Roanoke Rapids, NC. You will administer medications, monitor residents, and participate in care planning with the interdisciplinary team.

Qualifications

• Active LPN/LVN license in the practicing state.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in supervisory/charge nurse roles preferred.
• Familiarity with EHR systems and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physicians of changes.
• Develop and review individualized care plans with the team.
• Complete clinical documentation per policy; coordinate admissions/discharges.
• Participate in QA activities and regulatory surveys.
