Leisure Care is seeking an LPN Supervisor to join our team as a full-time clinical leader. You will oversee nursing activities, triage changes in resident conditions, and coordinate medication management in a warm, resident-centered environment.

This role requires 2+ years in assisted living or skilled nursing, with knowledge of geriatrics, dementia care, and state regulations. You will supervise staff and support the Health & Wellness Director on the floor to ensure exceptional care.

Qualifications

• 2+ years of assisted living or skilled nursing experience.
• Knowledge of geriatrics, dementia care, and state regulations.

Responsibilities

• Provide triage for incidents and changes of condition reporting to Providers and Responsible.
• Oversee medication management and coordinate care with vendors.
• Support the Health & Wellness Director in supervising and educating staff.
• Be a go-to health resource for residents, families, and team members.
• Assist staff and be present on the floor.
