Job summary

Rochester Regional Health is seeking a Licensed Practical Nurse (LPN) in Hemodialysis for St. Mary’s location. This role supports patient care under the supervision of an RN and focuses on dialysis treatment for adults with CKD.

Schedule includes days, evenings and weekends, totaling 20 hours per week. Responsibilities include assessments, assisting in treatment plans, documentation, IVP medications, and initiating/disposing dialysis accesses.

Qualifications

• Graduate from an accredited School of Practical Nursing OR ASN/BSN in Nursing.
• Current New York State LPN license.
• Completion of the NYS Department of Health and NYS DOE approved training curriculum for LPNs working in Outpatient Hemodialysis Units (completed as part of unit based orientation).
• Completion of the NYS DOH and NYS DOE approved annual competency for LPNs accessing central lines in a hemodialysis unit (completed as part of unit based orientation).
• Basic Life Support (BLS) Certification required.

Responsibilities

• Conducts and documents a thorough assessment of each patient’s medical status upon admission and throughout the patient’s course of treatment.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Complies with the evaluation, treatment, and documentation guidelines of the department.
• Acts in compliance with hospital, regulatory, and professional standards and policies.
• Maintains clinical currency by pursuing continuing education opportunities.
• Administers IVP medications specific to dialysis patient population.
• Initiate and discontinue dialysis treatments using multiple access sites including AV fistulas, AV grafts and Central Venous Catheters (CVC).
• Clean and redress CVC skin sites.
• Draw Blood from CVC.
• Respond to CVC line separations.

SUMMARY

As a Licensed Practical Nurse in Hemodialysis, your specialized skill set and ability to collaborate with the Registered Nurse is vital to exceptional patient care.

Rochester Regional Health offers a full range of Dialysis treatment options to patients with chronic kidney disease. We treat adults from people from multiple backgrounds, cultures and ethnicities. We care for patients temporarily, either their kidney function recovers or they return to their home dialysis center, move or get a transplant; or some are with us on an ongoing basis. Patients have dialysis treatment anywhere from 2-3 days a week, from 3 to 5 hours each treatment. Our staffing model consists of RN’s, LPN’s and Techs.

PHYSICAL REQUIREMENTS

Moderate - Clinical - Moderate clinical roles are patient facing roles that require continual movement, including standing, walking, and lifting 20-50 lbs., along with pushing and pulling medical equipment and assisting with patient positioning. Employees perform frequent bending, twisting, and reaching while completing fine motor tasks in busy or confined clinical environments. High sensory acuity is essential for monitoring patient status and responding to alarms or safety concerns. Emotional and cognitive demands are substantial, involving decision-making under pressure, responding to emergencies, and maintaining sustained focus. Environmental exposures include infectious agents, bodily fluids, chemicals, and radiation. TB testing, mask-fit testing, and physical exams are common requirements.
