Job summary

Frederick Living is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to residents and lead the neighborhood team. This role emphasizes person-centered care and supports professional growth of staff.

You will collaborate with nursing leadership, document care accurately, and participate in care plan reviews. Our community offers benefits, flexible hours, and opportunities to make a meaningful impact in residents' daily lives.

Qualifications

• LPN license in Pennsylvania and current registration.
• High school diploma or equivalent.
• BLS Healthcare Provider certification.
• Strong verbal and written communication skills.
• Basic computer skills and comfort with EHRs.

Responsibilities

• Observe and report changes in resident condition to Nurse Supervisor or physician extender.
• Communicate with Supervisor, ADON, DON, NHA, care providers, and family/POA as needed.
• Use evidence-based practices to ensure safe, high-quality care delivery.
• Assist in managing personnel through performance evaluation, coaching, and education.
• Complete all required documentation accurately and timely.
• Participate in resident transitions of care, care conferences, and care plan reviews.
• Collaborate with Cedarwood staff to ensure optimal resident outcomes.
• Comply with infection control practices.
• Maintain hand-held communication equipment for call bell management and team communication.
• Attend required trainings and team meetings.
• Abide by the organization’s code of conduct.
• Perform other duties as assigned.

Flex Pool – Hours and days vary. Rates starting at $34.25/hour plus shift differentials of $2.50 for evenings, $1.50 for nights and an additional $1.00 for weekends!

About the Role

• Complete all required documentation accurately and in a timely manner
• Participate in resident transitions of care, care conferences, and care plan reviews
• Collaborate with Cedarwood staff to ensure optimal resident outcomes
• Comply with Infection Control practices
• Ensure hand-held communication equipment is maintained for effective call bell management and team communication
• Attend and complete all required in-services, training, and team meetings
• Abide by the organization’s code of conduct
• Perform other duties as assigned

What We’re Looking For

• Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania
• High school diploma or equivalent
• Prior experience in geriatric nursing preferred
• BLS Healthcare Provider certificationBasic computer skills
• Strong communication skills, both verbal and written
• Ability to role model professional appearance and demeanor
• Demonstrates respect, integrity, compassion, and excellence
• Commitment to person-centered care initiatives
• Conflict resolution skills and ability to coach and support staff
• Flexible schedule and willingness to work extra hours as needed
• May be asked to assist in emergencies and resident evacuations
