Job summary

Qualifications

• Active Wisconsin LPN license (required).
• Strong clinical, communication, and patient care skills.
• Experience in long-term care or skilled nursing is highly desirable.
• Must be able to read, write and communicate in English.

Responsibilities

• Provide quality patient-centered, evidence-based care to residents.
• Perform initial data collection on all residents.
• Assist in implementing person-driven care plans as developed by RN.
• Utilize Relationship Based Care principles in daily duties.
